Dr. Elizabeth Stoebe was born and raised in Fort Dodge. She attended Sacred Heart Grade School and graduated from Saint Edmond High School. Dr. Stoebe graduated with an associate’s degree in nursing from Iowa Central Community College, and she received her bachelor’s in nursing from Grandview College in Des Moines, IA. She worked as a nurse in Surgical ICU and the emergency department at Mercy Hospital in Des Moines. She graduated from University of Osteopathic Medicine and Health Science in Des Moines, IA. Dr. Stoebe did family practice in Humboldt, Mason City, and Fort Dodge, and is board certified in family practice. Dr. Stoebe serves as the medical director of CHCFD. She is married to Kurt Stoebe, and they have two sons, John and Michael.

Dr. Janet Secor graduated from Fort Dodge Senior High, received her master’s degree in Spanish at the University of Northern Iowa, and her D.O. degree at the College of Osteopathic Medicine and Surgery in Des Moines. She began practice as a business partner of Sara Sutton, D.O. for six years, and then was in solo practice for five years. Dr. Secor joined Trimark Family Practice Group for around 15 years, then joined Community Health Center of Fort Dodge and retired from office practice in July of 2014. Dr. Secor still visits nursing homes and does occasional house calls. Her hobbies include gardening, bicycling, singing, history, and camping, along with various community activities.

Lisa Chase has been a certified family nurse practitioner with Community Health Center of Fort Dodge/Eagle Grove since 2012. She attended Graceland University and completed her clinical rotations in family practice, geriatrics, and pediatrics. Prior to completing her degree to practice, she worked as a registered nurse for several years in medical surgery, emergency department, orthopedics, and gynecology. Lisa enjoys seeing patients of all ages for health promotion, prevention, and care of acute and chronic illness. In her personal time, she enjoys traveling and family fun with her husband, Mike, and two sons, Cael and Porter.

Ann Feser, ARNP, joined the Community Health Center of Fort Dodge in December 2016. She graduated from Denison High School in 1993 and attended Iowa Methodist School of Nursing, graduating in 1997. Ann has worked in various fields of nursing during her career, including medical surgical, intensive care, cardiology, and emergency department. Ann completed her bachelor’s in nursing through Kaplan University. She attended the University of Cincinnati completing her master’s in nursing and nurse practitioner degree in 2012. Ann is a certified family nurse practitioner through AANP. She has worked in family practice and urgent care since starting her nurse practitioner career. Ann moved to Fort Dodge in 2007 with her husband, Jeff, and her daughters, Kaley and Taryn. In her personal time, she enjoys reading and spending time with family and friends. She welcomes patients of all ages for health promotion, prevention, and care of acute and chronic illnesses.

Terrilee has a heart for improving community health. She has worked in Iowa Community Health Centers since receiving her Master of Nursing degree from Clarkson College in 2014. She has experience with telemetry, labor/delivery, and providing care to veteran populations through the VA. Terrilee finds endocrine and metabolic disorders especially fascinating. She enjoys helping patients with PCOS, obesity, diabetes, and age-related hormone symptoms. She is one of the few providers in the state of Iowa with certification in Advanced Bio-identical Hormone Replacement Therapy. BHRT is a medical treatment method, which is used to treat people with dropping or uneven hormone levels. It is generally used to help treat conditions like cancer, osteoporosis, perimenopause/menopause, fibromyalgia, thyroid issues, and adrenal disorders among others. Terrilee also has certification from Cornell University in Plant-Based Nutrition and is passionate about using healthy nutrition to promote wellness and weight-loss to help her patients live their best lives. As a Family Nurse Practitioner, Terrilee provides a full range of primary care services for patients of all ages at our Mason City location. Terrilee is married to her husband, Peter, with whom she shares two grown children, Loralei and Owen. When Terrilee isn’t caring for her patients, she enjoys working with her church youth group, hiking, scrapbooking, and experimenting with vegan cooking!

Jennifer Pudenz, ARNP joined Community Health Center-Fort Dodge in January 2021. She received her nursing degree from Trinity College of Nursing, Moline, IL in 2000. Over the years she has worked in a variety of clinic settings including: medical/surgical intensive care, neonatal intensive care and newborn nursery. She obtained her Master’s of Science in Nursing from Clarkson College Omaha, NE in 2012 and is a board certified Family Nurse Practitioner. As a nurse practitioner she has worked mainly in family practice setting caring for patients of all ages, as well as urgent care. She has also worked as a massage therapist. She is married and has four children. In her free time, she enjoys spending time with her family and traveling.

Tayler Rasch is a certified family nurse practitioner with Community Health Center of Fort Dodge, joining the team in November 2022. Tayler is a Fort Dodge area native having been born and raised in Manson, Iowa. She attended Iowa Central Community College where she received her associates degree before attending Chamberlain University to complete her BSN and FNP. Prior to completing her degree to practice, she worked as a primary care nurse and taught Health Science for Fort Dodge Senior High School. She was also an educator with Iowa Central Community College where she taught Certified Nursing Assistants as well as clinical instruction. Tayler enjoys seeing patients of all ages for health promotion, prevention, and care of acute and chronic illness. In her free time, she enjoys simply spending time with her family and friends. She and her husband Landon share three children.

Bailey is a North Iowa native; born and raised in Charles City. She has over a decade of experience caring for patients in a variety of health settings such as family medicine, pediatrics, OBGYN, emergency medicine, cardiac step-down, and post-surgical units. She received her associate degree in nursing from North Iowa Area Community College in Mason City in 2013. Bailey went on to receive her Bachelor of Science in nursing degree from Walden University in 2018 and her MSN-FNP (Master of Science in Nursing, Family Nurse Practitioner) from Walden University in 2022 She and her husband of 16 years, Ben, have 4 children and 3 dogs. They enjoy spending time cheering on their kids in their various athletic activities. When the weather is warm you can find the Schutz family doing anything and everything outdoors such as biking, hiking, kayaking, and fishing. As a Family Nurse Practitioner, Bailey cares for patients of all ages for health promotion, prevention, and care of acute and chronic illness at our Mason City location.

Jennifer Zigrang graduated right here in Fort Dodge from ICCC with her RN in 2002. She has worked in many different specialty areas, but focused on emergency room nursing and OB/GYN. She graduated from Maryville University with her master’s degree in December 2015. She spent hundreds of hours in her clinical practicums caring for all ages of people–from newborn to the elderly, and everyone in between. She holds her advanced practice nursing degree and has board certification in family practice, which deals with all aspects of primary health care for all ages. Jennifer was born in Fort Dodge and has lived on the family acreage near Eagle Grove her whole life. She has been married since 1985, has two granddaughters, five German Shepherds, and one labradoodle, (plus one cat that lives in the bedroom for safety).

Dr. Erika Bowman is from Bloomington, Illinois. She received her bachelor's degree from Texas A&M University. While studying at Texas A&M, she met her now husband, Dr. Thaylan Bowman, during a genetics lecture. Dr. Bowman attended the University of Texas where she received her Doctor of Dental Surgery degree in Houston, Texas. After living in Texas for a decade, she and her husband missed the Midwest lifestyle and decided to move back home. She enjoys spending her free time reading, cycling, playing with their animals (cats for now, but excited for a dog in the future), and traveling.

Dr. Thaylan Bowman was raised in the Fort Dodge-Humboldt area. After graduating from Humboldt High School, Dr. Bowman relocated to Texas for 9 years where he obtained his Bachelor of Science degree at Texas A&M University and Doctor of Dental Surgery degree at UTHealth Houston School of Dentistry. He met the love of his life and fellow Midwesterner, Dr. Erika Bowman, while attending a genetics lecture at Texas A&M University. In 2023, the Bowmans decided it was time to come home and Community Health Center of Fort Dodge was the perfect fit for employment. Dr. Bowman enjoys reading, hunting, fishing, and staying up to date on his continuing dental education.

Kumari Saswati Kar has over 11 years of combined clinical experience as a general dentist in India and the United States. She graduated from dental school in India and then completed her advanced education in General Dentistry, along with a Master’s in Dental Sciences, at the Eastman Institute of Oral Health, University of Rochester, NY. After graduating from dental school, Dr. Kar worked in community clinics and hospitals, where she was actively involved in workshops aimed at improving oral health within communities. She has also worked in a community clinic in New York, where, in addition to providing comprehensive general dentistry care, she participated in several research projects focused on enhancing community oral health. Notably, her work included studies on oral health during pregnancy and its impact on early childhood caries. Dr. Kar is married, and in addition to her passion for dentistry and patient care, she enjoys cooking, hiking, camping, and traveling to new destinations with her husband in her leisure time.

Dr. Marcela Paulino DaCosta is from Hampton, Iowa and the second oldest of five kids. She is the first in her family to go to college and received her Bachelor’s degree and Doctor of Dental Surgery from the University of Iowa! After graduating from dental school in 2016, Dr. DaCosta and her husband moved to California but definitely thought Iowa was better so they moved back two years later. In her spare time Dr. DaCosta enjoys traveling, going to the movies and cuddling her two German shorthaired pointers while reading a book (preferably Harry Potter).

Kelsey joined the Community Health Center of Fort Dodge in February 2018. She graduated from the Iowa Central Dental Hygiene Program in May 2010. She and her husband, Adam, had their first daughter, Ivy, in July 2017. They enjoy visiting Kelsey’s parents in Montana, as well as doing anything outdoors and playing with their three dogs.

Sydney Hensley joined Community Health Center of Fort Dodge in March 2023. She is a graduate of the Iowa Central Community College Dental Hygiene program. Sydney grew up in Humboldt and currently lives in Fort Dodge with her husband and Berenedoodle, Llyod. She enjoys gardening in the summer and going on walks with her husband and dog.

Mary joined the Community Health Center of Fort Dodge in May 2022. Mary graduated from Iowa Central Community College Dental Hygiene Program in May 2018. Mary grew up in Humboldt, IA where she met her high school sweetheart and now husband Jeremiah. They have four fur-children. Three cats named Pete, Zeke, and Minnie and one dog named Remy. Mary enjoys skiing, painting, watching cooking shows, fishing, shopping, history, and having board game nights with family and friends.

Marissa has been with Community Health Center of Mason City since opening in 2018. She graduated from Newman Catholic High School and from there she moved to Minnesota to study Dental Hygiene. She then graduated from Normandale Community College with her degree in Dental Hygiene. She practiced for four years in Minnesota before deciding to move back home to Mason City. Marissa has two daughters; Grace, who was born February 2016, and Leona, born August 2018. She enjoys spending time with her girls and watching them grow! She also likes to shop, camp, and take vacations. It is her goal to get the girls to all 50 states before they are 18!

Bethanie joined Community Health Center of Eagle Grove in August of 2023. She is a graduate of the ICCC Dental Hygiene program. Bethanie grew up in Webster City where she still resides today with her husband Brett, Daughter Kennedy, two sons Royce and Trace. They have many animals including a Labradoodle, two cats and five chickens. When not at work Bethanie enjoys spending time with family, friends, camping, shopping, listening to audiobooks and going on vacation.

Roxanne joined Community Health Center of Fort Dodge in December 2018 as a mental health therapist. Roxanne completed her Bachelor of Arts in Family Life Education with psychology and music minors through Concordia University St. Paul. She then moved to Iowa and started working on a master’s degree in Clinical Mental Health Counseling from Walden University. She is licensed as a mental health counselor and a certified alcohol and drug counselor. She has had various experiences including milieu treatment for children with severe behavioral challenges and attachment disorders, inpatient and outpatient substance abuse treatment programs, community outreach, and community mental health. Roxanne enjoys working with clients of all ages, empowering them to achieve their goals. She has experience with LGBTQ +, dual-diagnosis, pregnancy/post-partum, children, adolescents, early and middle adult, and geriatric. She is trained in Alcohol and Drug Specific therapy, Dialectical Behavioral Therapy, Parent-Child Interaction Therapy with attachment focus (Iowa – PCIT), Trauma Focused Cognitive Behavioral Therapy, and various play therapy techniques. In her spare time, she enjoys spending time with family, reading, and music.

Brian Ayersman earned his associate degree in nursing from Mercy College of Health Sciences, his bachelor’s degree from Chamberlain college and his master’s degree from Walden University where he graduated with a specialty in psychiatry (PMHNP). Prior to earning his degree to practice he worked as a psychiatric nurse at the VA of Central Iowa for nine years. Brian has been a resident of Iowa his entire life. In his free time he enjoys golfing, listening to live music and attending sporting events.

Holly McClintock joined the Community Health Center of Fort Dodge in October 2017. She graduated from Allen College with a bachelor’s in nursing in 2009 and a master’s degree in psychiatric mental health nurse practitioner in 2012. She is board certified as a family psychiatric nurse practitioner, meaning she can treat both children and adults. She has a passion for mental health, and her work experience includes an inpatient psychiatric unit, community mental health centers, and a private practice office. She provides psychiatric evaluations, medication management, and brief solution-focused therapy for a wide range of mental health issues throughout the lifespan. Holly strives to listen attentively to the patient’s concerns, develop a treatment plan that best fits his/her needs, and answers any questions the patient has, all while assuring that the patient feels valued and comfortable. In her free time, Holly enjoys spending time with her husband and three children.

Eileen Meier is a board certified psychiatric nurse practitioner and an attorney. She has a certificate in traumatic stress studies from the Trauma Center in Massachusetts. She also was a prior sexual assault forensic nurse examiner for adults and children. Eileen graduated from the University of Iowa with a BSN; Johns Hopkins University with a Master in Public Health; University of Maryland with a Master in Science; and American University with a Doctorate of Jurisprudence. She has teaching experience in health care policy and international human rights, and was a former federal lobbyist in Washington, D.C for national health care groups. Eileen has worked in the U.S. and abroad on domestic violence issues in Bosnia, Nepal and Iraq, with children in the former Yugoslavia after the Balkan wars, and in various post-conflict countries. Eileen has experience treating patients age 6-90 with schizophrenia, depression, bipolar, anxiety, ADHD, substance abuse, post-traumatic stress disorder, children in foster care and individuals with prior sexual and domestic abuse. She is particularly interested in post-traumatic stress disorder in adolescents and adults and foster children. She hopes to develop a program in the local area for equine-facilitated psychotherapy for veterans and adolescents, and owns seven horses.

Deborah Price joined the staff of Community Health Center of Mason City in 2019. Deborah began her career as an Occupational Therapist with a focus on upper extremity rehab. She was commissioned as an active duty officer with the rank of Captain in the US Army Medical Corp at Madigan Army Medical Center in Ft. Lewis, Washington. Following her military discharge with commendation she attended Vanderbilt University. She graduated from Vanderbilt as a nurse practitioner with a specialty in psychiatry. Deborah later completed a certification in Family Practice through Allen College in Waterloo, IA. She has experience in multiple areas of healthcare including a year of training in consultation-liaison psychiatry while working in adult and geriatric psychiatry for a Mayo Clinic satellite hospital. She is a transplant to Iowa being originally from Tennessee. She loves living in Iowa and especially appreciates “Iowa niceness”.

Katie joined Community Health Center of Fort Dodge in January 2023. As a Psychiatric Mental Health Nurse Practitioner, she sees children, adolescents, adults and geriatric patients. She has been a nurse for 11 years and has worked in a variety of areas like ED, OB, Medical surgical, mental health and has been a transitional health nurse. She began her education at Iowa Central Community College obtaining her ADN in 2011 followed by her BSN from Chamberlain University in 2017. She completed her master's degree in psychiatric mental health from Walden University in 2022. Katie grew up in Nebraska and moved to Iowa about 20 years ago. She enjoys spending time reading books, playing darts and of course with her husband and four children.

Angela Crosby joined Community Health Center in Fort Dodge in August of 2022 as a mental health therapist. Angela graduated with her master’s degree in Mental Health Counseling in 2015 from Walden University. She has worked in many diverse settings, including residential homes, domestic violence and youth shelters, behavioral health urgent care, and community mental health. Angela is trained in dialectical behavioral therapy and approaches therapy from a non-judgmental, trauma-focused, eclectic place. In her free time, Angela enjoys family time, genealogy, competing in fast fiction contests, and long road trips.

Bonni joined Community Health Center in October 2020. She earned a Bachelors Degree in Social Work (BSW) and Certification in Gerontology in 1994 from the University of Northern Iowa (UNI), a Masters Degree in Social Work (MSW) in 1999 from the University of Iowa, and a Masters Degree in Holistic Health Studies (MAHHS) in 2011 from St. Catherine University in Minneapolis. She has worked with diverse clients in a variety of settings, including hospital, housing, group home, case management, home and community-based mental health services, and community mental health centers. She takes a holistic, client-centered approach to emotional health and wellness and has a special interest in working with trauma and mindfulness practice. She is trained in Dialectical Behavioral Therapy (DBT) and Lifespan Integration Therapies.

Kirsten Olson joined Community Health Center of Fort Dodge in January 2019 as a mental health therapist. Kirsten grew up in Gowrie, IA and graduated from Prairie Valley. She completed her bachelor’s degree at Simpson College in sociology and criminal justice and master’s degree at St. Ambrose University in social work. She has worked in a variety of settings including residential care, hospice, outpatient mental health, behavioral intervention services, domestic violence shelters, and youth shelters. Kirsten enjoys working with clients focusing on empowering them to make change of all ages. She has experience working with a variety of struggles in children, adolescents, early and middle adulthood, geriatric, family, marriage, and LGBTQ+. She is trained in cognitive behavioral therapy, motivational interviewing, dialectical behavioral therapy, sand tray play therapy, parent-child interaction therapy with attachment, aggression replacement training, and a variety of play therapy techniques. Kirsten enjoys spending time with family, reading, playing games, and everything Disney and Harry Potter.

Savannah joined Community Health Center in July 2023. She graduated with her Masters in Mental Health Counseling from Buena Vista University in August 2022 with a focus on substance use and trauma informed care. Savannah is a certified alcohol and drug counselor (CADC) and has a background working with adolescents and adults with co-occurring substance use and mental health disorders. Savannah is passionate about empowering clients to develop confidence, moving forward and creating change. She uses various evidence-based techniques such as motivational interviewing and cognitive behavioral interventions to support clients through life transitions, communication barriers, family dynamics, substance use, anxiety, and depression. In her free time, Savannah enjoys reading, exercising, quilting, and spending time with loved ones.

Linsey Albright tLMHC joined Community Health Center of Fort Dodge in the fall of 2023. Albright is a graduate of Buena Vista University with a bachelor’s degree in human services and a master’s degree in clinical mental health counseling. She has experience working with anxiety, depression, substance related disorders, PTSD, and other trauma-related diagnoses. Albright strives to create a safe, supportive environment where patients’ mental health needs are addressed without judgement.
