Community Health Center of Fort Dodge with locations in Fort Dodge, Mason City, Dayton, Eagle Grove, Clarion and opening soon in Spencer serves all people, whether payment for services would be made under Medicare, Medicaid, or CHIP; or the individual’s race, color, sex, national origin, disability, religion or age. No one will be denied access to services due to inability to pay, our discounted/sliding fee schedule is available based on family size and income.
We offer medical, dental, and behavioral health services for patients of all ages and in multiple languages, we have many bilingual Spanish speaking staff. We also have a social worker(s) who assists patients in finding community support services and a financial advocate who helps with eligibility and enrollment assistance as well as the sliding fee scale.
24-hour on-call coverage is available, and we serve several counties surrounding Fort Dodge as well as Mason City, Dayton, Eagle Grove and Clarion.
