Colleen Miller has accepted a new role at Community Health Center of Fort Dodge, Inc as Chief Care Management Officer (CCMO). Miller began her career at Community Health Center of Fort Dodge in June 2013 and most recently served as the Chief Operating Officer. She brings over 30 years of nursing experience, including expertise as a Certified Case Management RN to the position as well as strong operational knowledge and a deep understanding of patients and workflows.
In this new position, Miller will help lead the organization’s continued focus on health outcomes, access to care, and patient-centered services. Miller will be guiding the development and rollout of new Transitional Care Management (TCM) and Chronic Care Management (CCM) programs, important initiatives designed to better support patients after hospital visits and those managing ongoing health conditions.
