Community Health Center of Fort Dodge with locations in Fort Dodge, Mason City, Dayton, Eagle Grove and Clarion serves all people, whether payment for services would be made under Medicare, Medicaid, or CHIP; or the individual’s race, color, sex, national origin, disability, religion, age, sexual orientation, or gender identity. We accept most insurance and have a sliding pay scale for those who qualify.
We offer medical, dental, and behavioral health services for patients of all ages and in multiple languages, we have many bilingual Spanish speaking staff. We also have a social worker(s) who assists patients in finding community support services and a financial advocate who helps with eligibility and enrollment assistance as well as the sliding fee scale.
24-hour on-call coverage is available, and we serve several counties surrounding Fort Dodge as well as Mason City, Dayton, Eagle Grove and Clarion.
