Community Health Center Outreach Services

Each of our locations offer additional outreach services to those in and around the local area. Services include eligibility and enrollment assistance, health education, interpretation/translation, care management services, social worker consultations, referrals to community support services, and referrals to specialists. See below outreach specifics for Fort Dodge, Mason City and Dayton Community Health Centers.

Fort Dodge Area

Social Worker
Our in house social worker can assist with community resource referrals to housing, food, transportation and other emergency assistance programs.

Financial AdvocateFundraising
Community Health Center of Fort Dodge has a trained Certified Application Counselor to review your health insurance needs. Our insurance enrollment services are free and open to the general public as well as CHC patients. Our Fort Dodge Financial Advocate is a SHIPP Certified Counselor. SHIPP which stands for Senior Health Insurance Information Program is a free, confidential service of the State of Iowa Insurance Division. We are able to assist patients and non-patients of the Community Health Center of Fort Dodge with Medicare open enrollment including Medicare Part D enrollment. In addition, our Financial Advocate can assist you with setting up a payment plan for your healthcare services. Anyone can make an appointment or walk in during office hours to receive outreach assistance. To make an appointment call 515-576-6500. This assistance is free of charge.

Interpretation and Translation Services
We have many interpreters on staff to bridge the gap of language barriers for patients being seen by a provider or in need of information on the phone. Many of our printed materials are also available in multiple languages.

Community Outreach Activities
The Community Health Center of Fort Dodge participates in and leads many activities throughout the year within the community. We are happy to attend tabling events, speak with service clubs and more. We are a 501c3 non-profit and welcome donations throughout the year. We hold four main annual events each year:

Back to School Block PartyBlock Party
Held annually in August during National Health Center Week this event is free and open to the public. During the event children heading back to school can receive free well child checks, immunizations and sports physicals. The afternoon/evening also includes a free meal, kid's activities and information for families. Local businesses and organizations can get involved by participating or donating goods, services or money to off-set the cost of the event.

Socks for the Sole
Held annually in July/August this is a sock drive to collect new socks to distribute to school kids during the Fort Dodge Back To School Bash put on by Upper Des Moines Opportunity. Donations or socks or monetary donations to this project are welcome.

Annual Fundraising Event
The first Saturday in November a fundraiser is held (usually a costume style party/dinner) to raise funds for the health center. Funds are raised through event ticket sales, raffle ticket sales and through a live and silent auction during the event. Many local businesses support by being monetary sponsors, donating goods and services to the auction and by attending the event.

Coats for Kids
The need for warm coats for kids during Iowa's cold winters is large. This project collects new and gently used coats, boots, gloves, hats and snow pants for school age kids. This project started by Shimkat Motor Company has grown and so has the need for coats. Individuals, businesses and services organizations can get involved by donating winter items, monetary donations or being a drop-off site for items. The United Way of North Central Iowa and other local partners assist with the event. The afternoon/evening also includes a free meal, kid's activities and information for families. Local businesses and organizations can get involved by participating or donating goods, services or money to off-set the cost of the event.

5-2-1-0 initiative
The initiative, which is a community collaboration, will help bring Iowa Department of Public Health funding to Mason City to create healthier Iowa Kids. CHC has participated in a Healthy Eating Focus Group which accomplished a goal of implementing nutritional prescriptions into our clinic and distributed approximately $5,000 worth of healthy food vouchers to families in our community.

Dayton Area

Flu Shot Clinic
A flu shot clinic is held in Dayton each October giving area residents the opportunity to be vaccinated. The flu shot is free with most insurance plans or is just $25 for those without insurance.

Dayton Expo
Community Health Center of Dayton participates annually in the Dayton Expo where local residents can stop and visit with staff.
